HEAlth in ART Community Lead

We are looking for another team member who can help us make a difference

The HEalth in ART Community Lead, we will be responsible for mapping, bringing together existing provision and developing new creative arts provision in Wrexham. Working with the Community Care Collaborative (CCC provides integrated primary, community and social care in an accessible GP setting) we will provide creative therapeutic opportunities for the patients who currently access treatment or live in the catchment area. They focus on early intervention and preventative work and actively engages with the community to ensure a grass roots approach. This role will be based in Mold and Wrexham.

Part time (2 days a week), fixed term 9 months, may be extended subject to funding and performance.
